CareSys care planning software features an assessment of care that can be personalised for the individual and used to create a care plan document.
The care plan is made up of a number of elements, including the care needs, risk plans, medication, key workers and personnel information.
There are a number tools available, including the MUST tool, body map for wounds and other legions, diary for visitors, accident tracking, and health measurements.
A daily record can be produced to provide a comprehensive life story for an individuals care. This daily record links back to the care plan, and provides an analysis of future care needs.
