Advanced Health & Care's personalisation toolkit may be applied to each of our products, however our portal teams will readily integrate feeds to and from third-party applications as and where appropriate.
Across the suite, Advanced Health & Care provide tools that allow the service user, their family or registered advocates to record scored outcomes and free-text commentary to illustrate their perceived value of any care that they have been given. Where appropriate, service users may also make self-referrals or browse available services or care workers in order to request services, dates and times to suit themselves. Individuals may also check online to view their care plans and itinerary.
This approach presents the opportunity for great efficiency gains for service providers and delivers the enhanced management capability and outcome-based contracting that is needed by commissioners. Of course, the service user is greatly reassured by the fact that they may request the service that they need, when they need it and without having to know which service is responsible for its delivery.
